Client wanted a very clean and functional website which can be used by both Doctors and Patients to Order Medical Equipment.

The main challenge was to adjust a 6 pages PDF files into Order Form and make it flow and look good.

I also designed and coded a very useful Email which includes all the details of Order form.

The website is working smoothly.
